Clifton Park Pediatrics is focused on our patients and finding ways to improve your experience. In a move to better coordinate health for our Clifton Park community, we have decided to consolidate with another Community Care Physicians pediatric practice to form one practice and move to a new location in Clifton Park effective October 1, 2021.
Upon our consolidation on October 1, our new practice name will be Community Care Pediatrics – Clifton Park, and we will be located at 942A Route 146, Clifton Park.
We are excited for this move, and we are confident you will find it beneficial and convenient. Our new location on Route 146 is only 3 miles from our current location. You will receive care in the same area, with access to the same subspecialties in Clifton Park, and now you’ll have the support of a larger, consolidated team of medical staff to assist you when needed, particularly for sick visits. In addition, we will continue to offer weekend hours, telemedicine services, and also phlebotomy/lab services on-site in the practice.
If your child has an appointment booked on Friday, October 1 or later with Dr. Mitta or Dr. Puthuparampil-Mehta you will be seen at our new office. You will receive updated appointment reminders with our new address and phone number on it. To continue your care with us, you don’t have to do anything further other than visit our new location – 942A Route 146, Clifton Park – for your appointments on or after October 1.
