Make an Appointment
All of our health care providers are trained and certified in the evaluation and treatment of all aspects of pediatric health care. To make an appointment, simply call our office during normal business hours at (518) 371-8000. Patients can also schedule physicals and sick visits online through our website, or request appointments using our patient portal, mycareDOT™, powered by FollowMyHealth®.
You select your children’s primary care physician. Their primary care physician provides them with regularly scheduled medical care and will also provide care for illnesses or injuries that arise between scheduled visits.
If your child is ill, and their primary care physician is not available, another Pediatric provider in our practice will evaluate and treat him or her. If the problem requires further care, you will be given an appointment to follow up with your child’s primary care physician. Their complete medical record is available to each provider within Community Care Physicians, so all are informed with the information they need to care for your child.
Cancel an Appointment
If you must cancel an appointment, we ask that you do so at least one day before the appointment. We are happy to reschedule your appointment.
You can now also cancel an appointment using our patient portal, mycareDOT™, powered by FollowMyHealth®.
We call every patient’s contact phone number before their appointment to verify that they will attend.
