Community Care Pediatrics – Clifton Park is dedicated to providing our patients with high-quality, comprehensive pediatric care. We care for patients starting with your first newborn visit until your college years.

Our practice has been Recognized by the National Committee for Quality Assurance (NCQA) as a New York State Patient-Centered Medical Home (NYS PCMH). This designation affirms our commitment to the delivery of innovative, quality, compassionate healthcare and highlights our dedication to the wellbeing of our patients. 

Each member of our office plays a critical role in delivering care to our patients. Our physicians, advanced practice providers, and other healthcare professionals work together to meet your needs. Our office focuses on prevention and disease management and treating acute issues that arise. We have special interests in nutrition and obesity prevention, the management and prevention of anxiety, the care for children with disabilities, and general wellness through a healthy lifestyle.

We understand that sometimes you need an appointment right away for urgent medical issues. We want our patients to access our practitioners when needed for continuity of care. We have weekend video telemedicine hours from 10:00 am to 12:00 pm on Saturday and Sunday. If you have an urgent health concern that cannot wait until Monday, you can call the office at 8:00 am to speak with a nurse to schedule an appointment with one of our providers. We also have an on-call pediatrician always available for urgent needs. We are affiliated with our Urgent Care Center in Latham, NY, if you need an in-person visit when our office is closed.

Please note, calls Monday through Friday between 5:00 pm – 8:00 am and Saturday and Sunday 12:00 pm – 8:00 am are “after hours” calls.  One of our providers is available for urgent needs that cannot wait until the next business day. All chronic matters and medication refills will be addressed during our regular office hours, Monday-Friday, 8:00 am – 5:00 pm.
